Community Health Improvement Plan (CHIP)
What is a Community Health Improvement Plan (CHIP)?
A Community Health Improvement Plan (CHIP) is a broad, action-oriented strategic plan to improve the health of the community. A CHIP is developed collaboratively by the community and focuses on priority health issues. A CHIP includes 3-5 priority areas with goals, objectives, strategies and measures to address them.

- 11:00 am – 12:00 pm

Mental Health Working Group Information
- 3rd Monday of each month
- 12:00 – 1:00 pm
